Cognitive Behavioural Therapy is one of the most researched, evidence-based approaches in psychology — but when clients ask what it actually looks like in a session, the honest answer is: much less abstract than it sounds.

It Starts With a Thought, Not a Feeling

Most people come to therapy talking about how they feel — anxious, low, overwhelmed. CBT works by going one layer beneath that: what thought passed through your mind right before that feeling showed up? Often, clients haven't consciously noticed the thought at all — it happens too fast, and only the feeling registers.

Catching the Pattern, Not Just the Moment

A single anxious thought isn't the target — the target is the pattern. If you consistently interpret silence from a friend as rejection, or a mistake at work as proof you're incompetent, that pattern is worth examining, because it's shaping how you experience your whole life, not just one moment.

Testing Thoughts Like Evidence, Not Facts

A core part of CBT is treating your own thoughts as claims to be tested, rather than facts to be accepted automatically. If you believe "everyone thinks I said something stupid," we look at the actual evidence for and against that — not to dismiss your feelings, but to loosen the automatic grip a distorted thought can have.

Behaviour Change Reinforces the Thinking Work

CBT isn't just talking — it usually involves practical experiments between sessions. If anxiety has led you to avoid something, part of the work is gradually re-engaging with it in a structured way, because avoidance quietly reinforces the belief that the feared thing is unmanageable.

Why It Tends to Work Relatively Quickly

Compared to some other approaches, CBT is often more structured and time-limited, which is part of why it's so widely used — for anxiety, depression, OCD and more. That doesn't mean it's simple or superficial; it means the work is focused and practical from the first session.
